During an infection or a blockage, mucus or pus fills up the sinus. Fluid can only be moved out from the very top of the pool, and any sinus surface that is underwater cannot help pump fluid out. It becomes a tight balance between how fast the fluid is being made and how fast the sinus can pump it back out.
